The ReVital Cancer Rehabilitation Program was developed to help people affected by cancer prevent and/or restore life roles, build strength, stamina, physical function and mobility; and maintain/regain independence.

As a trusted partner for numerous health systems and independent oncology practices throughout the country, we know how to support patients and add value to your oncology practice. Our mission is to provide access to high-quality, comprehensive rehabilitation services that span the cancer journey, from diagnosis through survivorship or palliative care.

Our partnerships help patients gain access to the specialized rehabilitation services they need, and help oncology practices stay focused on detection, acute treatment, and saving lives.

The Cancer Care Team Philosophy

We believe patients have better outcomes when they have access to an interdisciplinary cancer care team.

Physicians, nurses, pharmacists, social workers and others – including physical therapists, occupational therapists and speech-language pathologists – all play a vital role in a patient’s care continuum.

Our specially trained ReVital Cancer Rehabilitation therapists can serve as a valuable member of your cancer care team.1 ReVital clinicians receive extensive training so they can address the complex needs of cancer patients. They have the specialized competency and knowledge to safely, effectively and compassionately deliver cancer rehabilitation care.

The ReVital Cancer Rehabilitation Program is designed to support value-based care initiatives such as CMMI’s Oncology Care Model. ReVital is research-based and aligned with NCCN guidelines.3 As an extension of your cancer care team, we can help your practice:

Improve Patient Satisfaction

We help people affected by cancer and its treatments to activate their own care and improve their quality of life. For many patients, it’s an appointment they actually look forward to each week.

Reduce Emergency Department Visits

ReVital therapists serve as another set of eyes, ears and hands for high-risk patients. From initial evaluation on, our therapists are here for your patients – helping to assess and manage pain, balance, fatigue and other issues that can lead to falls and unnecessary ED visits.4-6

Improve Pathway Compliance

Overwhelming fatigue, pain and numbness are common side effects of cancer treatment that can impact chemotherapy compliance. We strive to help patients stay “on pathway” by providing evidence-based interventions to improve their functional status, pain and stamina.

Improve Practice Productivity

ReVital therapists provide valuable patient education and can often address patients’ function-related questions in a relaxed, supportive atmosphere. Ultimately, this may reduce call-back volume at your practice, enabling oncology providers to spend more time focusing on acute treatment.

Partnership Process

We know your plate is full. That’s why we developed a five-step process to take the guesswork (but not the flexibility) out of partnering with ReVital. Here is a brief summary of how we work with oncology practices to bring goal-oriented, high-quality, comprehensive cancer rehabilitation services to patients.

Step 1: Discovery – We take a consultative approach to learning more about the needs of your patient population, practice and community. We share successes and lessons learned from other partnerships to help fuel further ideation and a collaborative dialogue.

Step 2: Partnership – Together, we confirm clinical and operational goals, discuss educational needs and identify key team members who will support the implementation process. We confirm dates and prepare staff for next steps.

Step 3: Implementation – We work closely with your team to operationalize key processes. We help your staff to leverage current IT platforms to identify appropriate patients for referral. We offer CE-accredited R.N. education, evidence-based cancer rehabilitation pathways, and unique patient education materials to support your team.

Step 4: Go Live – Whether it’s in several centers or a couple of pilot locations, our team begins to offer cancer rehabilitation services within your community. Together, we capture the data and outcomes needed to measure program quality and optimize results.

Step 5: Grow the Partnership – ReVital provides regular reporting on clinical and operational outcomes. We work to continue strengthening our partnership by supporting your practice, staff and patients.
